Eric Reimer is the CEO and Co-Chairman of the Board at Healthmap Solutions and also serves as a director of Imagine360 and CardioOne. Eric has over 30 years of healthcare services experience and leadership, focused on growing payer service and provider companies. In 2024, Eric was recognized as the Entrepreneur of the Year® US National Life Sciences and Health award winner by Ernst & Young LLP (EY US).
Prior to joining Healthmap, Eric was an operating partner at private equity firm Water Street, served as Chairman of Unified Women’s Care, OBHospitalist Group, and ELAP Services, and held various executive leadership roles at Magellan Health Services. As the former Chief Executive Officer of CareCentrix, the leader in home health benefits management, Eric broadened the management team and introduced innovative product offerings leading to a tripling of company revenues. Previously at Magellan Health Services, he led the Health Plan West Behavioral division serving 15 Blues plans; was Chief Growth Officer responsible for sales, strategy, and acquisitions; and was Chief Executive Officer of National Imaging Associates (NIA), where he led the division through 800% revenue growth over two years.
Eric received his bachelor's degree from Washington University and a master’s in business administration from the Wharton School of Business at the University of Pennsylvania.
Joe Vattamattam
Founder and President
Joe is responsible for leading the organization to align the company’s innovative product offerings with its value proposition, culminating in high-performing risk-bearing products. As a founder of Healthmap Solutions, he set the strategic vision in place, leading its growth from an early-stage company to a national market leader in kidney population health management.
Joe has broad and deep experience developing, underwriting, pricing, and managing value-creating healthcare programs across multiple niches within healthcare. Joe was critical to driving organic growth, product development, acquisitions, and business process improvement during his tenure as Vice President Medical Economics at CareCentrix, and as Director, Health Services at WellCare Health Plans.
Prior to WellCare, Joe was an investment banker at Wells Fargo (formerly Wachovia Securities), where he provided capital market and mergers and acquisitions advisory services to organizations in the IT Services sector.
Joe serves as a director on the board of Heritage Property and Casualty Insurance (NYSE: HRTG). He earned his undergraduate degree in Mathematics from the University of Florida and his master’s in Business Administration from Georgetown University.
Tom Gaffney
Chief Growth Officer
Tom oversees the company’s growth expansion strategy, client relationships, and sales and marketing. He aligns the company’s extensive strengths and internal resources to bring the most effective low-cost, high-quality programs to patients living with kidney disease; supports providers in practicing best-in-class, lower-cost medicine; and reduces spend and trend for payers.
Tom is a nationally recognized healthcare executive with more than 30 years of experience building and leading organizations that change traditional methods of high-cost, lower-quality healthcare services. An industry trailblazer, in 2006 Tom founded Sleep Management Solutions (SMS), the first Sleep Benefits Management (SBM) company in the country creating the industry’s first SBM healthcare space. Before joining Healthmap Solutions, he was the Chief Customer Officer at CareCentrix, where he managed the company’s client portfolio and Account Management functions.
Howard Shaps, MD, MBA
Chief Medical Officer
Dr. Howard Shaps is a board-certified emergency medicine physician with more than 15 years of healthcare management experience. Additionally, he has extensive experience leading Quality, Care Management and Utilization Management teams. He previously served as Deputy Chief Medical Officer at Centene Corporation, where he had direct oversight of medical management and supported the policy, vendor management, and clinical strategy teams. Prior to joining Centene, he was Deputy Chief Medical Officer for WellCare Health Plans, responsible for medical management, policy, and value-based care programs. Howard earned his bachelor’s degree in psychology from the University of Michigan, his Doctor of Medicine from the Boston University School of Medicine, and his MBA, with distinction, from the University of Louisville College of Business. He is also a diplomat of the American Board of Emergency Medicine.
Eli Wahesh
Chief Operating Officer
Eli Wahesh is a senior healthcare executive with over 25 years of operations and leadership experience. As Chief Operating Officer at Healthmap, Eli is responsible for operationalizing and optimizing Healthmap’s end-to-end Kidney Population Health Management program.
Before joining Healthmap, Eli was the Chief Operating Officer of CareMetx, providing patient support services for specialty drugs on behalf of pharmaceutical manufacturers and provider organizations. Prior to CareMetx, Eli served as EVP of Operations for eviCore healthcare (an Evernorth/Cigna company), where he helped to grow and scale the business to manage over 100 million members, completing 22 million medical reviews annually for over 90 health plan clients across the U.S.
Eli has led contact centers, clinical organizations, product operations, claims operations, systems integration, network management, client services, and account management. A military veteran, Eli served over a decade with the 75th Ranger Regiment and 82nd Airborne Division. He holds a bachelor’s degree from the Virginia Military Institute and an MBA from Duke University.
Mark Sprague
Chief Financial Officer
Mark is a senior healthcare executive with more than 20 years of leadership experience. As Chief Financial Officer of Healthmap, he is responsible for providing financial management across all aspects of the company and oversight of the Finance organization.
Most recently, Mark was Chief Financial Officer of a $2B Healthcare division at Magellan. In this role, he provided strategic financial leadership of the Behavioral, Specialty Health, Employer, and Federal lines of business, serving over 50 million members. He has also held executive roles in the Pricing and Underwriting and Corporate Development organizations while at Magellan. Prior to Magellan, Mark was the Head of Actuarial and Underwriting for Aetna International.
He is a Fellow of the Society of Actuaries and earned his Bachelor of Arts degree from the University of Connecticut.
Alison Gilligan
Chief Legal Officer and Executive Vice President of Strategic Solutions
Alison is a senior healthcare executive with more than 30 years of experience. Most recently she was Chief Legal and Strategic Solutions Officer at CareCentrix, a post-acute and complex chronic care management company. In her role at CareCentrix, Alison managed all legal matters, strategic alliances, mergers and acquisitions, provider network strategy, competitive intelligence, and corporate strategy.
Prior to CareCentrix, Alison held senior leadership positions with Patient Care, a large regional homecare company; L.E.K. Consulting, an international strategy consulting firm; and Mariner Health Group, Inc., a publicly traded company operating sub-acute care facilities and ancillary service providers nationally. Alison began her career at the law firm Wiggin & Dana as a corporate and mergers and acquisitions attorney specializing in the healthcare area.
Alison holds a Law degree from George Washington National Law Center, where she was an editor on the law review. She received both her master’s degree in Business Administration and bachelor’s degree from Lehigh University.
William Moore

William Moore serves as Healthmap's Chief Information Officer. William brings 30 years of technology leadership experience, including 17 years in healthcare. He has extensive experience in startup roles as well as large-enterprise leadership posts and is known for developing enterprise-enabling technology platforms as well as building and leading the teams that design and implement them. Prior to joining Healthmap, Moore spent 12 years as Chief Technical Officer for CareCore National and later eviCore healthcare, where he was responsible for all technology strategy and operations functions. He served as the company’s first technologist and led the technology group as the organization grew from 30 to 3,000 employees. Before that, he worked with AT&T, where he developed his enterprise solution ideology while working with their largest global customers.
Ashley Amar
Chief Compliance Officer
Ashley is responsible for Healthmap’s compliance with federal and state laws and regulations. She ensures that the Board of Directors, management, and employees follow policies and procedures, and our behavior meets the standards within the Code of Conduct. Her ability to develop, implement and administer a robust compliance program enables Healthmap to provide excellent service within the law.
Ashley specializes in healthcare law and has worked at both hospitals and other service providers. Ashley has expertise in healthcare regulatory law, business law and human resources. She has resolved matters with the Office of Civil Rights, Office of Inspector General, FDA and state regulating boards. Her experience handling Joint Commission Accreditations, Administrative Law Judge hearings and Corporate Integrity Agreements provides the Healthmap team with the compliance leadership needed as the organization grows. Ashley graduated law school with honors from Western State University and earned a Business Certificate.
Stephanie Toth-Manikowski, MD, MHS
National Medical Director
Stephanie Toth-Manikowski, MD, MHS is a double board-certified physician in internal medicine and nephrology. Dr. Toth-Manikowski serves as National Medical Director at Healthmap and practices nephrology in Chicago. She earned her bachelor’s degree in Biology with honors from Harvard College, her Doctor of Medicine from University of Illinois Chicago College of Medicine, her Master of Health Science from the Johns Hopkins Bloomberg School of Public Health, and her nephrology fellowship at Johns Hopkins. She is a member of the American Society of Nephrology and the National Kidney Foundation.
Laura Betten
Senior Vice President, Clinical Services
As Senior Vice President of Clinical Services, Laura Betten leads Healthmap’s provider-facing and clinical teams. Laura has over 12 years of experience in Managed Care, with a focus on quality improvement. Prior to joining Healthmap, Laura held positions at Anthem, Molina Healthcare, WellCare Health Plans, and Centene Corporation, most recently serving as Vice President of Quality Improvement for Centene. Throughout her career, she has demonstrated expertise in managing all aspects of clinical quality, including HEDIS and Stars performance measures, NCQA accreditation, risk adjustment, quality analytics, and contractual compliance.
Laura has a bachelor’s degree in nursing from Purdue University and has over 30 years of experience as a Registered Nurse. She earned her master’s degree in business administration and healthcare management from St. Leo’s University.

Steve serves as managing partner of Shulman Family Ventures, a private equity firm since 2008, and is the Co-Chairman of the Healthmap Board. He is the former chairman and chief executive officer of Magellan Health Services (NASDAQ:MGLN), where he spearheaded the company’s successful turnaround, restructuring out of bankruptcy, and diversification into the nation’s foremost specialty healthcare managed care company. He also led Prudential Healthcare through its major turnaround while serving as the company’s chairman, president, and CEO, culminating in the company’s successful sale to Aetna. He was the co-founder of Value Health, Inc. (a NYSE) managed care company specializing in behavioral health and prescription drug benefit management.
Steve currently serves as the Chairman of the Board of CareCentrix (privately held post-acute managed care company) and Quartet (privately held technology company focused on the integration of behavioral and physical medicine). He is also a director of VillageMD, MedImpact, Healthmarkets and Facet Technologies.
Steve served as Chairman of the Board of R1 (NASDAQ) healthcare revenue cycle management company, Health Management Associates (a NYSE listed national company that owned and operated 71 hospitals), where he successfully participated in a hostile takeover and engineered its sale to CHS for $7.6 billion, which closed in early 2014. He previously founded Internet HealthCare Group, a venture fund focused on the intersection of early-stage health care and technology companies. Prior to that, Steve served in senior executive positions at Cigna and Kaiser.

Ari is a Partner of Five Arrows Capital Partners (FACP) and a member of the Investment Committee.
Prior to joining FACP in 2017, Ari co-founded Diamond Castle where he was a Partner since 2005. Previously, Ari served as a Managing Director at DLJ Merchant Banking Partners, where he had been a principal and a Vice President since 1995. Prior to DLJ, Ari was a Vice President at Bankers Trust Company for three years. Ari started his career as an Associate at Banque Nationale de Paris.
Ari currently sits on the boards of The Stepping Stones Group, Zenith American Solutions, Averhealth, Juvare, TextHelp and Healthmap Solutions. Representative prior board appointments include Managed Health Care Associates, Beacon Health Options, Multi-Color Corporation and Suture Express.

Cherrill Farnsworth

Cherrill is an influential national expert on health care change and an entrepreneur who has started seven successful companies over the course of her career.
Cherrill is a regular speaker at business and health care symposia. Since 1984, she has been a regular spokesperson before the Medicare Payment Advisory Commission (MedPAC) and the Centers for Medicare and Medicaid Services (CMS). She has testified on the need to regulate quality in radiology at the House Ways & Means Committee and Congress. She is also a member of the National Quality Forum (NQF), which was formed by the U.S. Senate in response to the Institute of Medicine's report on medical mistakes.
Nationally recognized for her achievements, Cherrill has received the KPMG Peat Marwick High Tech Entrepreneur of the Year Award and the Ernst & Young Entrepreneur of the Year Award. The National Association of Specialty Health Organizations has named her “Most Influential in Specialty Health.”
Houston named her a Woman on the Move as early in her career as 1986. In 2015, she was elected to the prestigious Committee of 200, a membership organization of the world’s most successful women entrepreneurs and corporate innovators. Also, Cherrill is an active elected member of the Women Business Leaders of the U.S. Health Care Industry Foundation and in 2021 was honored by the City of Houston Chamber of Commerce, which added her to their Hall of Fame.
A graduate of Butler University, Cherrill received her degree magna cum laude and has served on the school's board of trustees.
Jonathan B. Gavras, MD, FCCP

Dr. Jonathan Gavras is a healthcare executive focused on promoting innovative changes to healthcare delivery. He is currently the Principle of JBGAVRAS CONSULTANTS, LLC, providing expertise throughout the spectrum of the healthcare industry. He is also currently serving on the Board of Directors for both Healthmap Solutions and Shearwater Health. Jon’s past positions have included roles as Market President, Specialty Pharmacy Leadership and President of a comprehensive laboratory benefit management company. Dr. Gavras was Florida Blue’s Senior Vice President, Delivery System and Chief Medical Officer and served as the founding President of Guidewell Health. He also served as Chief Medical Officer and head of Specialty Pharmacy for Prime Therapeutics. Prior to joining Florida Blue in 2006, Jon held several senior executive leadership positions at UnitedHealth Group and Prudential Healthcare. Jon transitioned to being a full-time healthcare executive after a successful clinical career in Pulmonary and Critical Care Medicine.
Jon holds a BA in Chemistry and Psychology from Case Western Reserve University and a Medical Doctor Degree from New York Medical College, and he is a member of Alpha Omega Alpha.

Dr. Trent Haywood is a leading healthcare executive who has been at the forefront of the healthcare industry’s transformation to value-based care and whose expertise in developing and leading best practices to improve quality, drive clinical innovation, and eliminate social determinants of health barriers to care has earned him industry-wide recognition. He is a board-certified internal medicine physician who brings over 20 years of healthcare management experience to the Healthmap Board. Trent is the founder of KNOWALITY, LLC, a venture services firm dedicated to accelerating market adoption of innovative healthcare services and products that improve population health.
Most recently, Trent served as Chief Medical Officer of the Blue Cross Blue Shield (BCBS) Association, where he was responsible for guiding medical policy and clinical programs for the association’s 36 member health plans. In addition, he served as President of the Blue Cross Blue Shield Institute, a benefit corporation focused on addressing the social determinants of health for BCBS plans.
Previously, he led national efforts to implement value-based healthcare as Chief Medical Officer at Vizient (formerly VHA, Inc.) and Deputy Chief Medical Officer for the Centers for Medicare & Medicaid Services (CMS). He earned a medical degree from the University of Illinois at Chicago and a Juris Doctor degree from Northwestern University. Trent is a graduate of Notre Dame University, where he earned a Bachelor of Science degree.

Oliver serves as the Managing Partner of WindRose Health Investors and has spent the last 24 years as a healthcare industry investor and advisor. Prior to joining WindRose in early 2006, he worked in Merrill Lynch’s healthcare investment banking group, where he was responsible for advising leading healthcare companies on mergers, acquisitions, and capital structure strategies. Oliver joined Merrill Lynch in 1994, and during his investment banking career, raised more than $6 billion for his clients and advised on M&A transactions with an aggregate value of $16 billion. Prior to investment banking, he was a healthcare-focused securities analyst in Merrill Lynch’s equity research department, where he was ranked as one of Wall Street’s “Up and Comers” by Institutional Investor’s All-America Research Team Survey.
Oliver currently serves on the board of directors of basys, MDsave, myNEXUS, Shearwater Health, Trumpet Behavioral Health and Vital Decisions. Outside of his professional work, Oliver serves on the board of the African Dream Academy Foundation and as a deacon of The Brick Presbyterian Church. He lives in New York City with his wife and two children. He holds a Bachelor of Arts degree from Bowdoin College.

David is a Partner at WindRose Health Investors. Prior to joining WindRose Health Investors in 2008, he worked in the Healthcare Investment Banking Group of Oppenheimer & Co. (formerly CIBC World Markets), where he focused on strategic advisory, mergers and acquisitions, and public and private company financings. David received his Bachelor of Science in Business Administration from The University of North Carolina at Chapel Hill. He currently serves on the board of directors of MDsave, Shearwater Health, and Traditions Behavioral Health.

Jake Stark has worked at WindRose Health Investors since 2016, where he has focused on investments in the services sector of healthcare, and currently serves on the Board of Directors at Shearwater Health, Trumpet Behavioral Health, and Healthmap Solutions. Prior to joining WindRose, Mr. Stark worked at UBS Investment Bank in the Leveraged Finance & Financial Sponsors Group, where he was responsible for leveraged buyouts, restructurings, recapitalizations, and capital markets across a range of industries. Mr. Stark received his Bachelor of Arts in Economics and French from Williams College.
Michael Hammond

Currently the managing director of H2C Securities Inc., Michael brings four decades of healthcare industry expertise to the Healthmap Solutions Board of Directors. In his career, he has counseled senior management and boards in the development of optimal business and capital strategies and led the execution of some of the leading transactions in the industry. His many clients have included Cedars-Sinai Medical Center (CA), Children's Hospital Los Angeles, Texas Health Resources, and University of Miami Health (FL), to name a few.
Prior to the formation of H2C, Michael co-founded Shattuck Hammond Partners, Inc. and was a member of its board and management committee. He also served as President and CEO of PricewaterhouseCoopers Securities when Shattuck Hammond was owned by that firm. Before that, he managed the healthcare investment banking businesses at Salomon Brothers and Morgan Stanley, and was a principal at Cain Brothers, Shattuck & Company. He began his career practicing law at Brown and Wood (now part of Sidley).
In addition, he sits on the boards of Traditions Behavior Health and Prevounce. Previously, he served on the boards of Precyse Solutions, Cymetrixm, Accretive Health (now R1), and Acclara Solutions, where he was Chairman. He also served for 17 years on the boards of Danbury Health System and Hospital, with terms as Chairman of the Board of both. He is currently a member of the board of several investment funds sponsored by Graham Capital Management.

Richard Popiel, MD, MBA

Richard Popiel, MD, MBA is a board-certified internal medicine physician and healthcare executive with over 27 years of experience leading patient-centric healthcare organizations. Throughout his career, he has used his medical and business expertise to help individuals navigate the complexities of the healthcare system while ensuring the cost of healthcare remains affordable to consumers. He is the Co-founder and Managing Partner at Xploration Health, LLC.
Richard previously served as HealthMap Solution’s first Chief Medical Officer and made significant contributions to the company's go-to-market growth strategy as well as clinical strategy. He served as the Chair of HealthMap's Scientific Advisory Board and successfully recruited both Nephrologists and Primary Care physicians. Richard was also the former Executive Vice President and Chief Medical Officer for Cambia Health Solutions, a Portland-based healthcare solutions company working to create a person-focused and economically sustainable healthcare system. Prior to joining Cambia, he served as President and Chief Operating Officer of Horizon Healthcare Innovations, a Horizon BlueCross BlueShield (BCBS) of New Jersey company. Before that, he served as Horizon BCBS of New Jersey’s Vice President and Chief Medical Officer and as Vice President and Senior Medical Director for The Permanente Company, a subsidiary of Kaiser Permanente. Other former Board and Advisory Board roles include Tabula Rasa Healthcare, BioIQ, and Medecision. He also was as an advisor to CareCentrix.
Richard serves on the Boards/Advisory Boards of several healthcare organizations, including Pager, Feel Therapeutics, Pearl Infusion, APOS, and Cara Care, all trailblazers in using technology and predictive analytics to seamlessly connect patients to the right care at the right time in the right place. He is also a member of the Board of Directors of The George Washington University Medical Faculty Associates, one of the largest medical groups in the District of Columbia. Richard is a graduate of The George Washington University Medical School and received his MBA from the Kellogg School of Management at Northwestern University.

Robert J. Cooper, MD, is an attending physician in the department of endocrinology at Holyoke Medical Center in Holyoke, Massachusetts. Dr. Cooper received his medical degree from Albert Einstein College of Medicine, Bronx, New York, and completed residency training in internal medicine at Long Island Jewish Hospital, New Hyde Park, New York. He completed two combined years of fellowship training in endocrinology at Long Island Jewish Hospital and Winthrop-University Hospital, Mineola, New York.
Dr. Cooper is a fellow of the American College of Endocrinology and American College of Physicians, and a diplomate of the American Board of Internal Medicine (ABIM) and ABIM subspecialty boards of endocrinology, diabetes, and metabolism. He is a member of the American Association of Clinical Endocrinologists (AACE), American College of Physicians (ACP), American College of Thyroidology, American Diabetes Association, American Thyroid Association, and the Endocrine Society.
Dr. Cooper specializes in all areas of endocrinology, including thyroid disorders; diabetes; adrenal, pituitary, and lipid disorders; disorders of metabolism; and osteoporosis. He is a former longstanding member of the Legislative and Regulatory Committee and also served on the Endocrine Board Review Committee of AACE. He has served on the Advisory Board for the Connecticut American Diabetes Association Annual Meeting, as an advisor to the Cardiometabolic Health Congress, and as Editor of MedStudy Internal Medicine Board Review.
Dr. Cooper was previously an investigator for several clinical trials in diabetes. He also lends his medical expertise to various organizations by serving as a disability peer reviewer, council member, and expert consultant. In addition, he is a member of various speaker/lecture bureaus.
John Danziger, MD
Nephrology
John Danziger, MD currently serves as National Medical Director at Healthmap Solutions. He received his undergraduate degree at the University of Pennsylvania, his medical degree from the Southwestern Medical School of the University of Texas, and a Master of Philosophy from Cambridge University in England, before completing his nephrology fellowship at Beth Israel Deaconess Medical Center in Boston. He is an assistant professor at Harvard Medical School and a staff nephrologist at Beth Israel Deaconess Medical Center. Dr. Danziger is active in patient care, medical education, and clinical research. He lives with his family in Brookline and enjoys swimming and rock climbing in his free time.

Dr. Forman is an experienced clinical cardiologist at The Christ Hospital in Cincinnati, Ohio, where he is director of preventive cardiology. His busy, full-time practice focuses on a proactive approach to avoiding adverse outcomes. Dr. Forman’s work emphasizes the importance of a healthy lifestyle to prevent cardiovascular disease, which is the chief source of morbidity and mortality in chronic kidney disease patients. A member of Healthmap’s Scientific Advisory Board since 2017, he enjoys the challenge and focus on improving outcomes of renal patients from a population perspective.
Dr. Forman earned and his bachelor’s degree in psychology from Washington University in St. Louis, Missouri, and his Doctor of Medicine from The Ohio State University College of Medicine in Columbus, Ohio. He holds board certifications in cardiovascular medicine and nuclear cardiology. Dr. Forman is recognized as a Fellow of the American Society for Preventive Cardiology and the American College of Cardiology.
Brian K. Lee, MD
Nephrology
Brian K. Lee, MD, is a board-certified transplant nephrologist and the medical director of the Kidney Transplant Program within the Abdominal Transplant Center, the clinical partnership between Ascension Seton and UT Health Austin. He specializes in caring for both adult and young adult patients with end-stage kidney disease who may benefit from a kidney transplant. Additionally, Lee is a professor in Dell Medical School’s Department of Internal Medicine. Lee earned his medical degree from the Royal College of Surgeons in Ireland. He completed both an internship and residency in internal medicine as well as a fellowship in nephrology/hypertension at MetroHealth Medical Center/Case Western Reserve University. He also completed a fellowship in transplant nephrology at the University of California, San Francisco, and a fellowship in integrative medicine at The University of Arizona to inform patients about the use of complementary and alternative medicine during the kidney transplant process.
Lee previously served as the medical director of the Living Kidney Donor Program at the University of California, San Francisco, where his team performed evaluations to get patients added to the kidney transplant wait list, evaluated prospective living donors, and ensured compatibility between donors and recipients. In his current role, in addition to helping complete presurgical workups, he provides care for patients after surgery and follows up with patients to manage immunosuppression and any other complications that may occur. If a patient’s kidneys fail after surgery, he helps evaluate the patient for a second transplant.
His research focuses on living donation, and he studies the barriers to donation, the outcomes of transplants from living donors, and ways to optimize kidney function and longevity following living donor transplantation. He also studies how to desensitize recipients (lower antibody levels) to lower their risk of a rapid, severe rejection of the transplanted organ. He is a member of the American Society of Nephrology, the American Society of Transplantation and the National Kidney Registry. Lee is a transplant nephrologist with UT Health Austin.
Lisandro Montalvo-Burke, MD
Nephrology
Dr. Montalvo is Healthmap’s Medical Director in Puerto Rico. He is a board-certified nephrologist and has been practicing nephrology in Canovanas, PR at his private practice since 2005. He is passionate about collaborating with providers from various specialties to improve kidney disease management while maintaining a patient-centric approach to care. He has served as an advisor and subject-matter expert for Quality Insights Renal Network 3 since 2014, and he is also an advisor for the Puerto Rico Emergency Preparedness and Response Activities Renal Coalition (PREPARAR-C), an organization that was formed after Hurricane Maria to ensure that dialysis patients would have access to care during emergencies. He has held consulting positions with health plans and dialysis centers on the island and is an advocate for value-based care initiatives in the field of kidney health.
Dr. Montalvo earned his bachelor’s degree in psychology from the University of Chicago and completed his Doctor of Medicine degree, internal medicine residency, and nephrology fellowship at the University of Puerto Rico School of Medicine. He is a member of the American Society of Nephrology, Renal Physicians Association, National Kidney Foundation, and Sociedad de Nefrología e Hipertensión de Puerto Rico.
Thomas D. Nolin, PharmD, PhD, FCCP, FCP, FASN
Pharmacy
Dr. Thomas Nolin is a tenured Associate Professor in the Department of Pharmacy & Therapeutics, and in the Department of Medicine Renal-Electrolyte Division at the University of Pittsburgh. Dr. Nolin’s primary research focuses on developing an understanding of the impact of kidney disease and renal replacement therapy (RRT) on nonrenal metabolism and transport pathways and corresponding effects on drug exposure and response. Dr. Nolin has a strong record of interdisciplinary and interinstitutional collaborations with regulatory, pharmaceutical industry, and academic scientists in the conduct of a broad range of clinical-translational and population health studies in patients with kidney disease. He has served as chair of the Kidney Health Initiative workgroup (a joint effort between the U.S. FDA and the American Society of Nephrology) to assess pharmacokinetics during RRT, as co-chair of the Extracorporeal Treatments in Poisoning (EXTRIP) Workgroup, and as an invited panelist on FDA advisory committees addressing nephropharmacology-related topics. Dr. Nolin has published over 150 manuscripts and book chapters and has presented more than 175 scientific abstracts and invited talks. He serves on the editorial board of the Clinical Journal of the American Society of Nephrology, and as an Editor of the textbook Pharmacotherapy: A Pathophysiological Approach. He is a Fellow of the American College of Clinical Pharmacy, the American College of Clinical Pharmacology, and of the American Society of Nephrology.
Colleen Ryan, MD

Dr. Colleen Ryan is a practicing primary care physician in Los Angeles, CA, at Private Medical, a full-service, multi-family office for health and medicine, providing evidence-based care to members in New York, Los Angeles, San Francisco, and Silicon Valley. Prior to Private Medical, Dr. Ryan’s work at Cedars-Sinai included leadership positions in the Department of Primary Care, as Physician Champion for Organizational Advancement, and as a member of the Board of Directors. Her clinical work at Cedars-Sinai included providing Executive Health and Concierge medical care. She began her medical career in Boulder, CO, working with under- and uninsured patients at Clinica Family Health, was Chair of Medical Services for Boulder Community Hospital, and later founded a private practice focused on personalized primary care. With her personalized and holistic approach, Dr. Ryan finds great joy in collaborating with her patients to help them achieve their goals. During her free time, she enjoys spending time with her family, practicing yoga, and getting outdoors to bike, ski, and surf.
Jenny Shen, MD
Nephrology
Dr. Jenny Shen is an Associate Professor of Medicine at UCLA and a board-certified practicing nephrologist at Harbor-UCLA Medical Center, a safety net hospital. Her research focuses on assessing and improving outcomes of patients with chronic kidney disease, particularly in the area of health disparities. Her work has included using large databases (“big data”) to improve outcomes at a population health level. She received her MD from Columbia University, completed her internal medicine training at UCLA, and completed her nephrology fellowship and earned a master’s degree in Epidemiology from Stanford University.
Tiffany Shiau, MD
Internal Medicine
Dr. Tiffany Shiau is a board-certified internal medicine physician and a certified Diplomate of the American Board of Lifestyle Medicine. She earned her bachelor’s degree in public health and master’s degree in cancer and reproductive biology from Johns Hopkins and graduated from Jefferson Medical College (now Sidney Kimmel Medical College). She completed her residency at Thomas Jefferson University and subsequently served on its faculty.
Tiffany has clinical experience with various practice models, and she currently practices at University of Pittsburgh Medical Center (UPMC), Central PA region. She is the physician lead at her practice and works directly with her regional medical group to improve operation and care delivery for their patients. She is passionate about providing individualized and comprehensive healthcare to her patients and is interested in impacting an even larger patient population.
